Disclosure of Outside Activities
The University of Florida requires that all faculty and staff submit a Disclosure of Outside Activities and Financial Interest Form.
In the event that a faculty or staff member does not participate in any outside activities with financial benefit, “no activities to report” must be written at the top of the form and the form must be signed and dated on the last page.
Once completed and signed, this form must be submitted to the Business Office.
