Students Registered for COVID Vaccine Clinic 05.21.2021

For students registered to receive the first dose of the Pfizer COVID-19 vaccine on Friday, May 21 at the Department of Health Alachua vaccine clinic hosted at P.K. Yonge, please review the Pfizer COVID-19 Vaccine Information Sheet.

For ON-CAMPUS students who have registered and who have their completed COVID-19 Vaccine Screening & Consent Form with them, the vaccination clinic will begin at 1pm (during the school day) and students will be directed to the clinic at the appropriate time.

OFF-CAMPUS students who have registered should plan to come to campus with completed COVID-19 Vaccine Screening & Consent Form at 3pm in order to avoid elementary pick-up traffic.

IMPORTANT!

— Students MUST bring a completed/signed COVID-19 Vaccine Screening & Consent Form to the clinic on Friday, May 21.

— Students who do not have a copy of the completed/signed form will not be able to receive the vaccine.

— Parents/guardians do not need to attend.

— Students will be monitored for 15 minutes after receiving the vaccine.
