Planning for 7th Grade 2021-22 | TDAP
March 17, 2021Families of 6th Grade Students — As you start to look toward your student’s 7th grade year, we remind you that Florida law requires 7th grade students receive a TDAP vaccination before their first day of 7th grade.

The required form is called a DH680 – Florida Certification of Immunization. Certification of Immunization Forms can be submitted to the P.K. Yonge Main Office or emailed to Nurse Eva Gonzalez | nurse@pky.ufl.edu
