Medication Pick-Up, May 4-8
April 30, 2020As the end of the school year approaches, this is a reminder for families to pick up all medications from the clinic from Monday, May 4 to Friday, May 8. Please note that P.K. Yonge requires all medication to be picked up by parents or guardians only.
Medication Pick-Up Dates: May 4-8, 2020
Campus Hours: 7:30am – 4:00pm
Procedure:
— Email mainoffice@pky.ufl.edu to notify the school that you will be coming by to pick up medication.
— When at the Front Gate, inform the Gate Guard that you are on campus to pick up medication and include student name.
— The Gate Guard will notify Front Office staff.
— Follow driving directions from the Gate Guard.
— Park in the Front Circle outside the Front Office.
— P.K. Yonge staff will meet you in the Front Circle – please stay in your vehicle.
— You will sign a form acknowledging you have picked up your child’s medication.
— Front Office staff will return your child’s medications to you.
Note for the New School Year
All new medications must be supplied for each new school year. Please remember medication authorization forms must be submitted at the beginning of each school year. For these forms and additional clinic information, please visit the Clinic page on our website.
