Informed Consent for Teacher Research

September 23, 2019

As many are aware, our teachers engage in a formal practice of closely examining their teaching practices, curriculum, and learning assignments to improve learning and outcomes for students.

Attached families will find a description of our teachers’ self-study work (known as teacher inquiry). See Parent/Guardian Informed Consent.

Parents/Guardians are not required to reply or sign consent their child’s participation as there will be no disruption to our typical education program.

If you DO NOT CONSENT to your child’s teacher using your child’s work samples (with all identifying information removed) in professional presentations, please sign and return the attached document to your child’s teacher or the front office by Friday, September 27, 2019.  Paper copies of the attached letter were distributed to all students today Monday, September 23, 2018.

If you elect at any time after September 27th to not permit teachers to use your child’s work (with all identifiers removed) please sign the attached document and return to your child’s teacher or the front office.  If you have questions, please don’t hesitate to contact me, Dr. Lynda Hayes, P.K. Yonge Director (lhayes@pky.ufl.edu).

Thank you for supporting our incredible teachers as they seek to improve learning for your student and for supporting the mission of our school.
