FluMist & COVID-19 Vaccine Clinic 10-12-2021
September 27, 2021This year, our on-campus vaccination clinic provided by the Florida Department of Health – Alachua will offer the FluMist and the COVID-19 vaccine. Both clinics will take place on campus on October 12.
FluMist & COVID-19 Vaccine Clinic, OCTOBER 12, 2021
All students (K-12) are eligible for FluMist.
Students 12 years of age and older are eligible for the COVID-19 (Pfizer) vaccine.
FluMist Form 2021-22 & COVID-19 Vaccine Form
Submit your consent form by Wednesday, OCTOBER 6.
Deliver by hand to the Main Office or the Clinic.
Email to nurse@pky.ufl.edu.
If you wish for your student to receive FluMist and or the COVID-19 vaccine on campus, please download, print, and complete: FluMist Form and COVID-19 Vaccine Form and return the forms by Wednesday, October 6. Forms may be delivered to the Main Office, to the Clinic, or emailed to nurse@pky.ufl.edu. Information regarding second COVID-19 vaccine doses will be shared on the day of the vaccine clinic.
Students K-12 will receive a paper copy of the FluMist form. Students 6th-12th grade will receive a paper copy of the COVID-19 vaccine form.
