FHSAA Forms Required for Secondary Sports

Families of Secondary Students — A reminder that in order to try out for and participate in sports, students must have a completed FHSAA Physical + Parent Consent Form.

FHSAA Physical + Parent Consent Forms for the 2021-22 school year can be picked up from the Main Office or downloaded from the Athletics page on the P.K. Yonge website. Some important dates for athletics are included below. Check web pages for athletic teams for new and updated information.
