COVID-19 Vaccine Clinic @ PK, May 21
May 13, 2021Great news! The Florida Department of Health Alachua will be offering COVID-19 vaccinations for P.K. Yonge for students aged 12 years and older on the P.K. Yonge campus on Friday, May 21 beginning at 1pm.
If you are interested in your student receiving the COVID-19 vaccine at P.K. Yonge on Friday, May 21, please review the Pfizer COVID-19 Vaccine Information Sheet, and download, complete and sign the attached COVID-19 Vaccine Screening and Consent Form. Students must bring the completed and signed form to the vaccination clinic on the day in order to receive the vaccine.
To help us plan the day, please let us know that you are planning for your child/ren to receive the vaccination at P.K. Yonge by submitting this form: P.K. Yonge COVID-19 Vaccination Clinic.
Parents/Guardians need not be present when students receive the vaccine. Vaccines will be administered and students will be monitored for 15 minutes before returning to class.
Students will be able to receive the first dose on campus and access local vaccination locations to receive the second dose. A vaccination record card will be provided to students once they receive their first dose. This vaccination record card must be provided at any vaccination location in order to receive a second dose.
