Reminder: 7th Grade TDAP Vaccination Required
July 27, 2021Families of Incoming 7th Graders — The law requires that 7th grade students have a TDAP vaccination before their first day of 7th grade (August 10). The required form is called a DH680 – Florida Certification of Immunization.

Students who do not have the required document will not be able to remain at school and must be picked up by a parent/guardian.
Forms can be submitted to the Main Office | mainoffice@pky.ufl.edu.
Students will not be permitted to return to school until they have submitted the required form.
