7th Grade: Required Immunizations
August 7, 2018Greetings P.K. Yonge Families of 7th Graders,
The law requires that 7th grade students have a Tetanus-diphtheria-acellular pertussis (Tdap) vaccination before their first day of 7th grade. The required form is called a DH680 – Florida Certification of Immunization.

7th grade students who do not have the required document will not be able to attend classes and will be placed in ISD until a family member can pick them up.
Forms can be submitted to the P.K. Yonge Front Office.
Students will not be permitted to return to school until they have submitted the required form.
