FluMist Vaccine Clinic
September 13, 2023This year, our on-campus vaccination clinic provided by the Florida Department of Health – Alachua will offer the FluMist vaccine on campus on October 16.

All students (K-12) are eligible for FluMist
FluMist Form 2023-24
Please submit your consent form by Friday, OCTOBER 6.
Deliver by hand to the Main Office or the Clinic.
Students will receive FluMist Forms at school. If you wish for your student to receive FluMist vaccine on campus, please return the FluMist Form by Friday, October 6. Forms may be delivered to the Main Office or to the Clinic.
Students K-12 will receive a paper copy of the FluMist form.
