New Student Registration and Activity Fee

Your child has been selected for admission at P.K. Yonge DRS. To confirm your acceptance, you must complete this form and pay the Activity Fee by the deadline stated in your letter of invitation (no extensions granted).

Failure to comply by the deadline will immediately rescind this invitation for admission and another applicant will be selected.

The following information is excerpted from the P.K. Yonge Admission Policy:

Activity Fees

The Activity Fee is $300.00 per student per academic year. $150.00 of the $300.00 is non-refundable. For new students the Activity Fee is due by the deadline indicated in the letter of invitation. In subsequent years, the Actvity Fee is due in two installments, one by October 31 and one by April 15.

Reduced Activity Fee schedule is based on eligibility guidelines established by the Federal Free or Reduced School Meals program. Those applying for a reduced Activity Fee must submit a current 1040 Federal Income Tax Form that lists names of the children for whom the reduction is requested. If granted, reduced Activity Fees will be adjusted, per student per year to $150.00.

Refund Schedule: For families accepting admission/enrollment to P.K. Yonge who have paid the full $300 Activity Fee and later terminate admission/enrollment:

I have read and understand the information above regarding Acceptance of Admission and Activity Fee.
Activity Fees are $300 per student per year.
I must submit $300 by the deadline provided in my letter of invitation to ensure enrollment.
I have read and understand the Refund Schedule above.
My student's subsequent enrollment may be terminated or revoked if I have provided incomplete, inaccurate, or false information; if I have withheld any information; or if the student applicant cannot or does not comply with P.K. Yonge's Attendance Policy.

Form submissions and student spaces are confirmed only when payment is made.
