Parent/Guardian Authorization to Release Information, Photos, Video and/or Digital Recordings
Throughout the school year, the P.K. Yonge Developmental Research School (“P.K. Yonge”) and the University of Florida highlight student achievement, activities, and events at P.K. Yonge. This Media Release Form authorizes P.K. Yonge and the University of Florida to use, record, photograph, share, and publish certain information regarding your child for educational, promotional, and informational purposes.
By granting consent below, you permit P.K. Yonge, the University of Florida, and their employees, agents, or representatives to record, publish, and use photographs, video, audio, and/or digital recordings of your child, as well as your child’s student work, for any legal purpose, including but not limited to:
- school publications or productions (newsletters, yearbook, brochures, athletics, performing arts, etc.)
- advertising, marketing, or promotion
- P.K. Yonge and/or University of Floria social media pages (Facebook, Instagram, YouTube, etc.)
- file sharing sites (e.g., Google Drive, Dropbox, OneDrive)
- news releases, broadcasts, and print media covering school activities
You understand that neither you nor your child will receive any payment, compensation, or royalties, in any form, for the use of these materials. You further understand that all photographs, videos, and recordings produced by P.K. Yonge and/or the University of Florida remain property of P.K. Yonge and/or the University of Florida.
By consenting to and signing this release, you release P.K. Yonge and/or the University of Florida, and their agents, employees, or other representatives from any liability, known or unknown, arising out of P.K. Yonge’s and/or the University of Florida’s use of this material.
This consent may be revoked at any time by submitting a written notice to the assistant director of communications at P.K. Yonge. (Note: Such revocation, if any, shall apply prospectively from the date received and cannot apply to materials already printed or published.)
