THE ORTHOPEDIC SPORTS MEDICAL INSTITUTE IS OFFERING FREE ATHLETIC PHYSICALS ON THURSDAY, APRIL 25TH FROM 8:00PM-9:00PM.  FORMS MUST BE FILLED OUT BEFORE AN ATHLETIC PHYSICAL WILL BE CONDUCTED.  FORMS CAN BE DOWNLOADED HERE.

Athletic Physical Policy

NEW FORMS ARE HERE!

All students who wish to participate in athletics must have a completed FHSAA physical and FHSAA parent consent form turned in to the athletics department before conditioning/practice. P.K. Yonge  has combined the FHSAA athletics physical, the FHSAA parent consent form and the heat and concussion form to create a color coded form that is easy to track.  

The 2012-2013 form color is PINK.  The form may be picked-up from the front office, the business office, Kelly Barrett or John Clifford. Physicals and parent consent forms are valid for 365 days. Forms can be turned into the Kelly Barrett at the front office.

Insurance Information

Families who wish to purchase personal insurance for their student athlete can purchase through the Fowinkle Insurance Agency.


We Appreciate Our Parent Volunteers!

Our Athletics programs could not function without you. Thank-you for volunteering at our school. Please complete a Volunteer Application every year.  The school encourages that you have an application on file in order to participate in any school organization.  We made the form on-line fill-able so all you need to do is fill it, print it, and turn it in to the front office.


College Bound Athletes?

High School athletes who plan to play at a Division I or Division II college/university need to be familiar with the NCAA updated Rules.
